Water Rights Applications/Forms

Forms can be downloaded from this website for Underground Water Applications, for Surface Water Applications, and for various other applications or forms.

If you have questions as to how to fill out any of the sections on an application, please contact the District Office that administers the groundwater basin or stream system in which the application is located.

The following forms can be used in the following ways:

  • Fill out the form(s) online (MS Word version) and print
  • Download the form(s) to your computer (MS Word version), fill out electronically, and print
  • Print the form(s) off the web (PDF version) and fill them out by hand
